Decubitus (Pressure) Ulcers
Pathogenesis: result when pressure over the skin exceeds arteriolar pressures, leading to local tissue hypoxia
Risk factors: immobility, incontinence, nutritional compromise
Staging:
- I: intact skin with non-blanchable erythema - treat with protective film and preventive measures to avoid progression
- II: Loss of dermis, with shallow open ulcer or blister - treat with occlusive dressing to maintain moist environment and facilitate healing
- III: Full thickness tissue loss with visible fat - treat with debridement and appropriate dressing (absorptive or moisturizing, depending on wound)
- IV: Full thickness tissue loss with visible bone, tendon, or muscle - treat as stage II
