Multiple Endocrine Neoplasia (MEN) 1

 

MEN 1 Refresher: "3 P's"... pituitary, pancreas, parathyroid tumors
Autosomal Dominant predisposition to tumors of the parathyroid glands (present in nearly all by age 50 years), anterior pituitary, and pancreatic islet cells. Most pituitary lesions are adenomas, which makes it hard to associate the clinical presentation of AI with her this reported history. There have been some reports of adrenal adenomas MEN1, but again this would not cause her current presentation


ZE Review:
1/3 of MEN1 patients have clinically apparent pancreatic islet cell or gastrointestinal endocrine cell tumors of which ZE is the most common. Gastrin hypersecretion in the Zollinger-Ellison syndrome in MEN1 may lead to multiple peptic ulcers or diarrhea. VIPomas also occur with MEN1 and may cause diarrhea. Our patient had no observed diarrhea while inpatient although she reported up to 20 stools/day on ROS.
